When should a patient be advised of their records being on a database? - patient database

I know that the authorization for the release of this information to give, but when exactly should be informed about the existence of computerized databases containing medical information?

Let us assume that all medical records and every little thing that is put into a database. I did not say you do not just shared.

there is no reason why the patient should be informed of your data is a database that most health care in the country transition to a paperless system with all the graphics and the entire team. I see no reason why should anyone in particular for their success is the electronic be informed and stored in a database.
